Minimally-invasive approaches are preferred in the surgical management of large ovarian cysts. Gaining abdominal access can pose significant challenges for the gynecologic surgeon, especially when trying to minimize spillage of cyst contents. This video reviews a combined approach between mini-laparotomy and laparoscopy to remove a 20 cm ovarian cyst using a protector-retractor system. This approach maximizes abdominal access at the umbilicus, while allowing the procedure to be otherwise be performed laparoscopically and facilitating specimen retrieval.
